Nonadrenergic noncholinergic (NANC) vasodilator mechanisms may contribute to the maintenance of adult pulmonary and systemic vascular tone. However, their actions in the neonatal circulation have not been studied. We aimed to investigate NANC vasorelaxation in neonatal and 2-week-old piglet pulmonary and mesenteric arteries and to examine the potential role of nitric oxide (NO) in this phenomenon. Responses to electric field stimulation (EFS, 50V, 0.25-32 Hz) were investigated in pulmonary and mesenteric artery rings (external diameter 150-200 µm) precontracted with the thromboxane A2 mimetic U46619, in the presence of guanethidine (10 µM) and atropine (10 µM). Under these conditions, EFS resulted in a frequency dependent relaxation of newborn pulmonary (maximal relaxation of 53±9.1%), mesenteric (68.8.2±7.1%) and 2-wk-old mesenteric (46 6.3%) arteries but this relaxation was significantly reduced (4.5±2.2%) in 2-week-old pulmonary arteries. In neonatal pulmonary arteries, the neurotoxin tetrodotoxin (0.3 µM), the NO synthase inhibitor L-NAME (0.1 mM), and the guanylyl cyclase inhibitor ODQ (10 µM) abolished EFS-induced relaxations, suggesting that NANC relaxation of porcine neonatal pulmonary arteries is mediated by NO, which is probably neuronal in origin. However, The expression in pulmonary arteries of the neuronal NO synthase (nNOS), as determined by Western-blot analysis, increased with postnatal age whereas the expression of the endothelial NOS (eNOS) did not change. In conclusion, NANC relaxation is present in neonatal pulmonary and mesenteric arteries and it is, at least partially, mediated through NO. NANC relaxation of porcine pulmonary and mesenteric arteries decreases with postnatal maturation.
Aim of study. To find correlation between the umbilical serum cord magnesium concentration in a group of newborns without antenatal exposition to magnesium sulphate and the neonatal outcome as well as to compare the neonatal outcome of babies with maternal magnesium sulphate treatment. Results. In a group of 82 newborns (31-34 GA; birth body weight <1500 g) the umbilical cord serum magnesium concentrations was assayed. Three subgroups of neonates were set apart: first group (N) with a normal magnesium level (n=28), the second group (n=19) with magnesium concentration <0.75 mmol/dl (D), and the third group (Mg) consisting of 35 newborn antenatally exposed to magnesium sulphate. These groups were similar (no statistical significances) in terms of birth body weight, gestational age, Apgar score and umbilical blood pH. Gradual decrease in magnesium concentration in relation to increase of gestational age was observed. In groups: D, N, Mg, gradual reduction of hospitalization time (p<0.05), risk of death, time of respiratory support, time of oxygen therapy, rate of bronchopulmonary dysplasia, periventricular leucomalation (p<0.05) were observed respectively. Likewise, a rate of serious neurological complication (IVH/ PVL) in group D was higher in comparison to groups N and Mg (although without statistical sig-nificances). Conclusion. Our results confirm that umbilical cord magnesium concentration in VLBW neonates have an influence on neonatal outcome. Although it is premature to recommend ordering maternal magnesium sulphate treatment to improve neonatal outcome, in our opinion the data presented here should at least induce magnesium concentration monitoring in pregnant women and magnesium deficiency correction.
Congenital candidiasis is a severe complication of candidal vulvovaginitis. It occurs in two forms, congenital mucocutaneous candidiasis and congenital systemic candidiasis. Also newborns are in age group the most vulnerable to invasive candidiasis. Congenital candidiasis should be considered as an interdisciplinary problem including maternal and fetal condition (including antibiotic therapy during pregnancy), birth age and rare genetic predispositions as severe combined immunodeficiency or neutrophil-specific granule deficiency. Environmental factors are no less important to investigate in diagnosing, treatment and prevention. External factors (e.g., food) and microenvironment of human organism (microflora of the mouth, intestine and genitalia) are important for solving clinical problems connected to congenital candidiasis. Physician knowledge about microorganisms in a specific compartments of the microenvironment of human organism and in the course of defined disorders of homeostasis makes it easier to predict the course of the disease and allows the development of procedures that can be extremely helpful in individualized diagnostic and therapeutic process.
Nowak J.: Insulin and glucose concentration changes in newborn piglets after sucking the colostrum from insulin administered sows. Acta Physiol. Pol. Immuno reactive insulin (IRI) concentration in sows colostrum has been previously proven to be much higher than that in blood. The experiment was carried out to show the influence of endogenous and added insulin in sows colostrum on insulinaemia and glycaemia of newborn piglets. In colostrum collected from 3 control and 5 experimental sows before loading, basal insulin concentration were 1.595 and 1.365 nM⁻¹ , respectively, and calculated for all 8 sows together were 1.451 nM⁻¹ (SEM ± 0.289). Basal plasma insulin concentrations calculated for 68 healthy piglets before sucking were little differentiated (mean 0.318 ± 0.044 nM I⁻¹ ), whereas glucose initial concentrations for those piglets (mean 3.581 ± 0.275 nM I⁻¹ ) were highly differentiated. Intramuscular loading of 5 experimental sows with insulin (80 I.U. per animal) caused an increase in the concentration of insulin in colostrum from 1.365 to 3.449 nM I⁻¹ (0.01, P < 0.02). The mean insulin level (0.313 + 0.04 nM I⁻¹ ) in experimental piglets blood plasma (n = 42) increased significantly to 1.234 + 0.07 (P < 0.001) after suckling by sows loaded with exogenous insulin. Glycaemic response of those two piglets litters was poor but showed a statistically significant increase (P < 0.001). The glucose concentrations in blood plasma samples of the other three litters did not changes after sucking. The experiment excluded the hypothesis that high level of insulin in colostrum could be the cause of hypoglycaemia in healthy piglets after sucking.
The aim of the study was to estimate the acceleration of birth parameters: body length, body weight, head and chest circumference of Lublin newborns as a response to environmental changes observed since 1964 till 2001. The environmental conditions changes were characterized by: health care standards, average consumption of selected foodstuffs, furnishings, level of education and income of households. The systematic, statistically significant increase of birth length was observed within the whole studied period. Improvement of indices, expecially concerning health care, in front of an average income decrease in Lublin voivodship comparing to Poland in the same period, should be emphasized.
Lipopolysaccharide (LPS, endotoxin) is the component of the cellular wall of Gram negative bacteria. Endotoxemia (sepsis) could produce multiorgan failure and in the early period of life LPS are responsible for the changes of metabolism and for the reduction of protein synthesis. The influence of neonatal endotoxemia on the pancreas at adults has not been investigated yet. The aim of this study was to assess the pancreatic exocrine function in the adult rats which have been subjected, in the neonatal period of life, to chronic LPS pretreatment. LPS from E. coli or S. typhi at doses of 5, 10 or 15 mg/kg-day was administered intraperitoneally (i.p.) to the suckling rats (30 g) during 5 consecutive days. Three months later these animals (300 g) were equipped with pancreato-biliary fistulae for the in vivo secretory study. Amylase release from isolated pancreatic acini obtained from these rats was also assessed. Pancreatic tissue samples were taken for histological assessment and for the determination of gene expression for CCK1 receptor by RT-PCR. Pancreatic amylase secretions stimulated by caerulein or by diversion of pancreatic-biliary juice to the exterior (DBPJ) was significantly, and dose-dependently reduced in the adult rats which have been subjected in infancy to chronic pretreatment with LPS from E. coli or S. typhi, as compared to the untreated control. In these animals basal secretion was unaffected. In the rats pretreated with LPS in the suckling period of life caerulein-induced amylase release from isolated pancreatic acini was significantly decreased, as compared to the untreated with LPS control. This was accompanied by dose-dependent reduction of mRNA signal for CCK1 receptor on pancreatic acini. Neonatal endotoxemia failed to affect significantly pancreatic morphology as well as plasma amylase level in the adult rats. We conclude that neonatal endotoxemia reduces gene expression for CCK1 receptor and could produce impairment of the exocrine pancreatic function at adult age.
Seven skulls of newborns and of infants aged 3 weeks to 12 months were studied using computed tomography. Size of the maxillary sinuses was measured. Between 3rd and l2nd week of life the ethmoidal sinus formed small spaces in ethmoidal labyrinth. Maxillary sinus could be noted at the level of lower orbital margin. In newborns, it formed a shallow indentation in the lateral wall of the nasal cavity. The anteroposterior dimension of the sinus was greater- than the two remaining dimensions. Beginning from the 3rd month of life, number of ethmoidal cells increased, maxillary sinuses enlarged and entered the body of the maxilla. Between 6th and l2th month, the mediolateral and the superoinferior dimensions of the sinuses increased. Small air spaces were also seen in the part of sphenoid bone, corresponding to sphenoidal sinueses.
The purpose of the present study was to check the infl uence of rural and urban environments on some of the parameters of lipids metabolism in the cord blood serum in healthy newborns, as well as the evaluation of the concentration of lipids, lipoproteins and apolipoproteins in the cord blood serum with relation to risk factors of atherosclerosis in the family of the studied newborns. The study included 75 newborns (37 from rural areas and 38 from urban areas). Newborns weight and length were recorded at birth. On the basis of the family history taken from the mothers, the atherosclerosis risk factors were established in the families of the studied newborns. In all of the studied newborns, concentration of triglycerides, total cholesterol and LDL, VLDL and HDL cholesterol as well as of apolipoproteins (apo-AI, apo-B) in the cord blood serum sampled soon after birth were performed. No statistically signifi cant differences between the mean levels of triglycerides, total cholesterol and cholesterol of fractions: LDL, VLDL, HDL and apolipoproteins (apo-AI, apo-B) in the cord blood serum in the newborns from rural and urban areas were found. However, when analysing the concentration of parameters of lipid metabolism in cord blood serum in newborns with regard to gender, higher concentrations of total cholesterol, LDL cholesterol and apo-AI in female newborns from rural areas, and higher HDL cholesterol and apo-AI in female newborns from urban regions were confi rmed. When analysing the concentration of lipids and lipoproteins in cord blood serum in newborns from families with risk factors confi rmed as compared to the families without that risk, both in the rural and urban regions no signifi cant differences were confi rmed. The studies have not proved any signifi cant differences between the levels of lipids, lipoproteins and apolipoproteins in the cord blood serum in newborns from rural and urban areas.
