Background. There is currently many over the counter products on the market that exert nutritional or physiological effects on the human body. The differences between dietary supplements and non-prescription drugs are however poorly understood by the average consumer and may thus affect their expectations as to the desired effect produced on the body. Objectives. To evaluate patients’ knowledge and attitudes towards dietary supplements as compared to non-prescription drugs. Materials and Methods. Subjects were 335 patients of the Mazowiecki Voivodeship Hospital in Warsaw, Poland. The data were collected from a face-to-face interview using a single and multiple choice questionnaire with 10 questions on dietary supplements. Statistical analysis used the Chi-square (χ2) test. Results. The majority of respondents were found to be familiar with the term ‘dietary supplements’, but had difficulties in classifying these products into appropriate categories. Over 55% do not consider dietary supplements to be foodstuffs and more than 40% considered such products to be drugs. Most respondents thought that the main purpose of taking dietary supplements is to improve nutrition, but over one third expected them to also treat disease. Over 70% declared taking notice to which category the non-prescription products they bought belongs to ie. whether non-prescription drugs (medicinal products) or dietary supplements. Conclusions. Many patients mistakenly believe that dietary supplements are drugs and can be used to treat disease and health disorders.
Background: Dietary supplements (DSs) are concentrated sources of nutrients and/or other substances, whose purpose is to supplement the normal diet. Their consumption in Poland has been on the rise, leading to numerous concerns. However, current legislation does not provide any background for a relevant pharmacovigilance system. Therefore, little is known about their adverse effects (AEs) or patterns of use in Poland. To shed some light on this, we conducted a nationwide survey. Aim of the study: To assess the prevalence of, and the reasons for, the use of dietary supplements in Poland via national survey with a collection and an analyse of supposed adverse effects of dietary supplements. Material and methods: The study was conducted using an online survey based on the original questionnaire. In this report, we present the results of the analysis of the first 200 responses. Results: As many as 55.5% (111) of respondents were ‘definitely sure’ or ‘rather sure’ about DSs safety. However, 70.5% (141) believed that they could have negative effects. Over 3/4 (153) of respondents used DSs themselves, and 61.0% (122) used them within last 12 months. Of those using DSs, 11.1% (17) reported diverse Aes, with GI tract irritation and mental disturbances being the most common (experienced by 35.3% (6), and 17.6% (3) of those reporting AE s, respectively). Conclusions: Respondents had mixed feelings about DS safety. Despite that, they used these products frequently. Of a note is high incidence of adverse effects experienced by DS users. This undoubtedly points to the need for a relevant pharmacovigilance system.
Background. A great variety of dietary supplements and fortified products, containing folic acid, are widely available to satisfy the market demand for supplementing the diet with this vitamin. Objective. To assess folic acid intake from dietary supplements and fortified products in students. Materials and methods. The study was performed between October 2010-April 2011 on 314 university students aged 19- 34 years, excluding pregnant or lactating women. Self-administered questionnaire was used to collect information about health, lifestyle and use of dietary supplements during previous month (frequency and doses) and frequency and portion of the fortified products consumed. Results. Diets were supplemented by folic acid by almost 13% of respondents; the average intake was 148 ± 99.7 μg/person/ day, (range 10-400 μg). Products fortified with this vitamin were used by 89% students with an average intake of folic acid 83.7 ± 101 μg/person/day, (range 0.3-660 μg); about a quarter of these subjects were however unaware that they were consuming fortified products. Both product types were used together by 11% students. Among these subjects the average folic acid intake was the highest (233 ± 151 μg/person/day, range 12.3–678 μg); with 66% originating from supplements and 34% from fortified products. There were no instances of folic acid being consumed above the tolerable upper intake level (UL), of 1000 μg/day. Conclusions. It is necessary to warn the public that overconsumption of folic acid, (through dietary supplements, enriched food), may be hazardous to health, since many people combine such multiple products.
Background. Concurrent use of dietary supplements and drugs may result in complications of pharmacotherapy due to possible interactions between their ingredients. Objectives. The aim of the survey was to estimate the intake of dietary supplements in a group of women over 50 and to analyse the risk of interactions between the ingredients of dietary supplements and drugs taken by the women. Material and Methods. The study was carried out among 146 women over 50 years of age. Questionnaire included detailed questions on the type of prescription drugs, OTC (over-the-counter) drugs, and dietary supplements taken. The risk of interactions was determined on the basis of chemical composition of the drugs and supplements specified by the manufacturer, by comparing the obtained data with literature reports on known interactions. Results. The analysis has shown that 88.4% of respondents constantly took prescription drugs, 44.5% of them took OTC drugs, and 66.4% of respondents took dietary supplements throughout the survey period. It has been found that 71.3% of surveyed women taking prescription drugs, took dietary supplements as well. Among women taking supplements and drugs, 36.9% of respondents were taking them concurrently, 60.9% kept such an interval, but only 21.8% of them waited for at least two hours. It has been found that the drug-supplement interactions might occur in 35.8% women under the survey. Conclusions. The analysis of the obtained results has revealed that taking dietary supplements by the group under survey was frequent, and the risk of interactions between dietary supplements and drugs was significant. It is recommended that doctors ask their patients about taken supplements during regular check-ups, and inform them about possible interactions between dietary supplements and drugs. Moreover, appropriate would be to change the labelling of dietary supplements, so that the packaging provides information on possible interactions between their ingredients and drugs.
The study aimed at evaluation the effect of dietary supplement of taurine on the oxidation-reduction status in riding horses, and especially on the extent of oxidative DNA degradation in lymphocytes.Ten Thoroughbred and half-bred geldings aged 6-13 years were classified according to breed and amount of work done into two groups – control (C, n=5) and experimental (E, n=5), the latter fed the diet with addition of 40 g taurine/horse/day. Blood samples were withdrawn from the horses’jugular vein before commencing the riding season and then after 30 days of working. In the blond some selected morphological and biochemical indicators were determined including the TBA-RS and 8-oxo-dG in lymphocyte DNA. It was found that physical effort of horses, being used for 30 days in recreation riding, affected homeostasis of redox status, and especially the rate of oxidative DNA degradation of lymphocytes. The addition of taurine to feed caused smaller oxidative stress,manifested by lower concentration of TBA-RS in plasma and of 8-oxo-dG in lymphocytes. The taurine lowered the lipid peroxidation intensity that occurred in horses due to the oxidative stress caused by physical effort. Furthermore, taurine revealed affinity to nucleic acids, counteracted the oxidative DNA degradation and decreased the damages of DNA in lymphocytes.
U ogromnej większości ludzi starszych, a mianowicie u tych, którzy mieszkają w domu problemy żywieniowe są raczej natury jakościowej niż ilościowej, przynajmniej w krajach uprzemysłowionych. Inna grupa ryzyka odznacza się niemalże przeciwnym problemem, na przykład większym zapotrzebowaniem na energię, a także na niezbędne składniki odżywcze. Niemożność zaspokojenia tego zapotrzebowania przyczyniła się do powstania koncepcji niedożywienia szpitalnego, która została zastosowana i opisana w kilku krajach. Wielu pacjentów, o których się mówi, że cierpią z powodu niedożywienia szpitalnego, faktycznie choruje z powodu niedożywienia spowodowanego chorobą podstawową. Jednak u wielu pacjentów szpitalnych etiologia niedożywienia jest wieloczynnikowa, z elementami zarówno szpitalnymi jak i chorobowymi. Ciężkie stany niedożywienia można łatwo zdiagnozować zestawem metod klinicznych i laboratoryjnych. Jednak przypadki z pogranicza wykryć jest trudno. Sama choroba może spowodować pogorszenie apetytu, zaburzenia żołądkowo-jelitowe, a nawet wymioty. Ponadto schorzeniom nowotworowym, a także innym stanom, towarzyszy zwiększenie metabolizmu oraz wysokie zapotrzebowanie na energię. Farmakoterapia może spowodować pogorszenie apetytu. Upośledzenie fizyczne lub/i umysłowe może zaburzyć spożywanie posiłków. Niedostateczna aktywność fizyczna jest przyczyną niskiego zapotrzebowania na energię, a tym samym powoduje trudności w uzyskaniu dostatecznego pokrycia zapotrzebowania na niezbędne składniki odżywcze. Nieodpowiednie otoczenie towarzyszące spożywaniu posiłków może być przyczyną niedostatecznego odżywienia. Niedożywienie może się zdarzyć także w przypadku złego stanu jamy ustnej oraz niedostatecznej wiedzy i zrozumienia problemów żywieniowych przez personel medyczny. W badaniach przeprowadzonych przez naszą grupę wykazano, że poprawa np. otoczenia towarzyszącego spożywaniu posiłków, a także wzbogacanie składnikami odżywczymi przyczynia się do poprawy stanu odżywienia tych pacjentów.
Background. Epidemiological studies indicate that by consuming 6-14 mg lutein daily, the risk of acquiring eye diseases like age-related macular degeneration (AMD) or cataracts becomes reduced. Their symptoms can also by such means be alleviated and treatment improved. Objectives. To estimate dietary intakes of lutein obtained from foodstuffs and supplements along with determining its main sources in selected groups of adults suffering from eye disease and healthy controls. Material and Methods. The study was performed in Warsaw and its neighbourhoods during 2008-12. Subjects were 375 adults aged 50-97 years, of whom half had been diagnosed with AMD and/or cataracts; constituting the test group. Dietary intakes of lutein were assessed by Food Frequency Questionnaire Method whilst interview questionnaires assessed the intake of supplements. Results. Overall, the average dietary intake of lutein from foodstuffs was 2.5 mg daily, with the test group being significantly higher than healthy controls (2.9 vs 2.1 mg daily). Women’s intakes were also higher than in men (2.9 vs 2.1 mg daily), as were those possessing higher or secondary education compared to the others with primary or vocational education (2.7 vs 2.3 mg daily). Fresh vegetables were found to be the main dietary sources of lutein that included green leafy vegetables and frozen vegetables, constituting respectively 63% and 13% of the dietary intake. Dietary supplements containing lutein were taken by 109 subjects of whom most had eye disease (over 80%); where the average daily consumption of lutein from this source was 6.5 mg. Conclusions. For older people, the dietary intake of lutein from foodstuffs may be insufficient to prevent eye disease. Taking daily dietary supplements would thus be indicated to make up such deficiencies of lutein.
Background. Mineral supplementation is important whenever any dietary deficiencies of these nutrients occur. However, an excessive consumption of such products can lead to the negative health consequences. Objectives. To estimate the mineral supplements contribution to total dietary intake for adults living in Warsaw. Materials and Methods. Within the frame of the EHES JA Poland and WAW-KARD Projects a representative sample of Warsaw population aged 20 years and above was screened in years 2011/2012. Nutrient intake and supplements use were available for 573 men and 497 women. The minerals levels were analyzed in reference to the Recommended Dietary Allowance (RDA) and the Tolerable Upper Intake Level (UL). Results. In Warsaw population the intake of calcium, magnesium and potassium were lower than RDA, and supplementation didn’t eliminate deficiency of these nutrients (except for magnesium in a group of women). Phosphorus intakes were somewhat higher than those recommended. Enriching the diet with zinc and copper supplements in both sexes were unnecessary, as was iron in men, as their daily dietary intakes closely met the recommended standards. Iron supplementation in women however proved effective, as their daily dietary intakes were rather low. Conclusions. The effectiveness of supplementing the diets of adults living in Warsaw with minerals depends on their type.
Background. Calcium is one of the most important minerals for the human body which is essential for healthy bones and teeth. Vitamin D has hormone-like properties in the human body. It is supplied with the diet, but it is also synthesized by the body under exposure to UV radiation. Vitamin D controls calcium and phosphorus metabolism and is responsible for bone modeling and mineralization. Objective. The objective of this study was to evaluate eating habits and food preferences of school-age children meet the demand for calcium and vitamin D, and estimate the frequency of consumption of foods rich in these nutrients. Material and methods. A total of 197 parents of 7- to 9-year-olds attending randomly selected primary schools in Lublin and Świdnik were asked to fill out a questionnaire designed by the authors. The results were processed by the Chi-squared test in the Excel application. Results. Considerable differences in the consumption of milk and dairy products were observed between age groups. In the group of 7-year-olds, 57.5% of children drank milk and ate dairy products at least once a day, whereas in the group of 9-year-olds, only 16.6% of children ate dairy products at least once a day. The intake of foods rich in vitamin D was equally low among the surveyed children. Conclusions. School-age children may be at risk of calcium and vitamin D deficiencies due to low consumption of milk, dairy products and fish, and inadequate vitamin D supplementation. Parents should be educated about nutrition to change the family’s eating habits.

Seven commercial food supplements present on the Polish market were evaluated for their in vitro antioxidant capacity. The selected products were in the form of hard gelatin capsules. They contained the extracts from chokeberry, cranberry, blueberry and green tea. The mixture of vitamins and minerals as well as the product containing vitamin C in substantial dose were included into comparison. The products were examined using three methods in order to evaluate their antioxidant capacity: electron paramagnetic resonance (EPR), Trolox equivalent antioxidant capacity (TEAC), oxygen radical absorbing antioxidant capacity (ORAC) assays. Total polyphenolic content was determined according to Folin-Ciocalteu method. The results were calculated per capsule. All studied preparations showed antioxidant properties and may provide substantial antioxidant protection. The in vitro antioxidant capacity varied considerably and was associated with the content of polyphenols in the capsule. The supplement with 250 mg of green tea extract was the most potent antioxidant in all assays. Nevertheless it must be remembered that the amounts of extracts were different in encapsulated products. As the quality of extracts and their properties are miscellaneous, there is a need to standardize dietary antioxidant supplements with respect to their antioxidant capacity if effective doses are to be recommended.
Autism is a neurodevelopmental disorder with symptoms arising that are apparent throughout the patient’s lifespan. Autism Spectrum Disorders (ASD) are characterised by impaired social and communication interactions as well as restricted, repetitive interests and behaviour. Currently in Poland, about 50 000 people suffer from autism, of which 1/5 are children. Epidemiological studies show that the incidence of autism is increasing, which may be due to the diagnostic category of ASD having been developed. Of vital importance in the treatment of autism, is early diagnosis which is conducive to more rapidly improving the quality of patients’ health. It is believed that both genetic and environmental factors may affect the development of the disease. Moreover, expert opinion emphasises the importance of making an adequate diagnosis when the first symptoms of autism start appearing which can be both psychological, gastro-intestinal and metabolic ones. Conventional treatment is based on the combination of behavioural and dietary therapy together with pharmacotherapy. For example, adapting an appropriate diet could help alleviate the disease severity, as well as the psychological and gastrointestinal symptoms. Much scientific research has indicated that pathogenesis of autism may have a beginning already in foetal life. During pregnancy, specialists should take special heed of metabolic disorders, which can increase the risk of ASD in children. One of the dietician’s tasks are to properly assess the nutritional status of mothers before and during pregnancy, thereby allowing changes in nutrition to be made wherever necessary in order that metabolic indicators be improved. Thus an important part of autism therapy is the improving patient’s nutritional status to prevent the onset of gastrointestinal symptoms. Adopting diets and tailored to individual disease symptoms, is linked to the nutritional requirements and food preferences of the patient. Specialists also emphasise that continual monitoring of the diet and nutritional status of children with ASD is required. It is also essential to start adequate dietary management in autistic patients with overweight, obesity or wasting, caused by improper nutrition. Frequently only a dietary therapy is insufficient to effectively treat autism. Many studies demonstrate the need to supplement the nutritional deficiencies of autistic patients with fatty acids omega-3, probiotics, vitamins and minerals in combination with medical and psychological interventions. A properly designed elimination diet adapted to the patient’s individual may also lead to relief of the autism symptoms and the occurrence of gastrointestinal disorders. Parents and caregivers should therefore be aware of the benefits of nutritional therapy and need for proper monitoring the treatment of patients with ASD. A review of nutritional factors, dietary treatments and diet supplementation in patients with ASD is presented.
