There has recently been an increase in surgical interventions to the inferior temporal lobe. The aim of the present study is to examine the anatomical structure and relations of the anterior choroidal artery, which extends to this region. A mixture of latex and ink was injected into the internal carotid and basilar arteries of 15 brains from fresh cadavers. In 18 out of 30 cases (60%) the anterior choroidal artery arose from the posteroinferior aspect of the internal carotid artery, in 8 (22.2%) from the posterolateral aspect and in 4 (2%) from its anterior part. The diameter of the anterior choroidal artery was 0.94 mm on average (0.7–1.2) and the average distance from the posterior communicating artery was 5.3 mm (3.8–8 mm); its distance to the bifurcation of the carotid was found to be 4.0 mm on average (2.2–8 mm). The cisternal segment of the anterior choroidal artery and the optic tract formed a neurovascular bundle. The branches arising from the plexal segment supply the lateral geniculate body, the thalamus and the optic tract. The resulting knowledge of the neurovascular relations of the anterior choroidal artery provides a safe surgical approach to the inferior temporal lobe.
Breast cancer is one of the most common types of cancer among women. The most common form of treatment of this illness is a surgical intervention consisting of a partial or radical mastectomy. This article describes the psychological impacts of this experience on the frequency and occurrence of complications in the sexual activity of women and to determine the role that medical staff can play in helping patients cope with these challenges. The loss of a breast can have negative effects on a woman’s emotional state, specifically in terms of feeling feminine and the relationship with her partner. This is often reflected in a reduced quality of life. The care extended to post-mastectomy patients should routinely include an assessment of possible sexual dysfunctions and monitoring of how such dysfunctions are coped with. The PLISSIT model makes it possible to indicate how post-mastectomy patients may be effectively supported by medical staff. It serves to define a group of patients requiring specialist help. It also aims to initiate a conversation about the difficulties of functioning in this sphere, to provide general information and change existing perceptions, to give specific advice on making referrals to a specialist, and to consider these types of existing problems. The described intervention model is applicable to individual work, as well as to work with couples and groups. This method depends on the type of intervention desired and on the current psychophysical state of the patient and her readiness to start a conversation concerning sexual activity.
The infratemporal fossa is often the site of pathology or surgical intervention. We describe an unusual muscle found during the routine dissection of the right infratemporal fossa. The literature germane to this variable muscle, best described as a variant of the pterygoideus proprius, is reviewed. The clinician may contemplate the wide array of muscular anomalies within the infratemporal fossa when considering unexplained neurological symptoms attributed to branches of V3 and pursue appropriate diagnostic testing.
The material for this study comprised 5 dogs between the ages of 5-9 months and 1 cat aged 1.5 years brought to the I.U. Veterinary Faculty Surgery Department Clinic between the years 1996-2005, with the inability to use their hindleg. The cases were presented to the clinic within a few days following the occurence of the lesion. Case histories revealed the causes to be jumping from a height in 4 animals and unknown in 2. A diagnosis of avulsion fracture of the tibial tuberosity was reached following clinical and radiological examination of the cases. Surgical intervention was carried out in all cases, of which fixation was achieved with a screw application in 5 cases and the tension band method using double Kirschner pins and cerclage wire in 1 case. In the clinical and radiographical follow-up approximately 2-4 month after the operation the trouble-free recovery of the patients was established in the light of the information given by the patient owners. There was no problem with respect to the limbs bearing weight in any of the cases. Therefore, no need was seen to remove the fixation material. In conclusion, the authors hope that it will be beneficial to veterinary practice to present the successful results achieved in the fixation of avulsion fracture of the tibial tuberosity using screws in 5 cases and the tension band method in 1 case, as well as reporting the rarely encountered lesion in cats.
