At the end of February 1959, during the second phase of the influenza pandemie in Poland, a number of quite atypical cases were observed in the Kłodzko district. On 7 March the first case of trichinellosis was diagnosed and very exact epidemiological investigations were immediately taken up which, however, were made difficult by the influenza epidemie. 378 cases of trichinellosis were diagnosed and 70 dubious cases established. They originated mainly from 3 places as well as from many other ones the contact of which with the first three places was established. 904 patients from the whole country stayed at this time in the sanatoria of the Kłodzko district but it was not yet possible to get data about any trichinellosis cases in this group, except 2 reports from Lódź and Wrocław. The detailed epidemiological investigations carried out are described, due to which the source of the infection could be established: It was raw sausage ("metka"), produced on the 30 January and in a quantity of 191 kg distributed to a number of places. As in the establishmetns of meat products all swine slaughtered are submitted to trichinoscopy, the imperfect method of trichinoscopy might be considered the reason of the epidemie.
In the group of 37 patients who went through acute trichinellosis chronic phase of the disease was observed for 9 months. The disease charts and results of examination from the pre-infection period being available, it could be found that the complaints given in anamnesis and revealed by examinations are due to T. spiralis infection. Particular attention was paid to changes in the motoric (muscular and osseo-articular) and circulatory systems. In spite of appreciable complaints from the motoric system, occasionally interfering with professional activity, no clear-cut abnormalities could be elicited in the objective examination. Striking was the consistence of the stated complaints as compared with other examined groups of patients but all persons showed cardiac affections (e.g. changes in the heart tones), and in half of them there were ECG abnormalities; in 5 persons the shortened electrocardiographic prednisolone test was negative. Deltacortril (Pfizer) or Azulene (Homburg) therapy was applied in 24 cases; the results obtained were encouraging though of short duration, the period of treatment being probably too short and the doses of the drug too small. Pathogenetic considerations on acute and chronic trichinellosis are given, with emphasis on allergic phenomena and topography of allergy, as well as on biochemical changes in the connective tissue, which in Hauss' studies are referred to as "unspecific reaction of the connective tissue".
