Recent literature data suggests that parasitic and fungal diseases, which pose a threat to both human and animal health, remain a clinical, diagnostic and therapeutic problem. Attention is increasingly paid to the role played by natural microbiota in maintaining homeostasis in humans. A particular emphasis is placed on the possibility of manipulating the human microbiota (permanent, transient, pathogenic) and macrobiota (e.g., Trichuris suis) to support the treatment of selected diseases such as Crohn’s disease, obesity, diabetes and cancer. Emphasis is placed on important medical species whose infections not only impair health but can also be life threatening, such as Plasmodium falciparum, Echinococcus multilocularis and Baylisascaris procyonis, which expand into areas which have so far been uninhabited. This article also presents the epidemiology, diagnosis and treatment of opportunistic parasitoses imported from the tropics, which spread across large groups of people through human-to-human transmission (Enterobius vermicularis, Sarcoptes scabiei). It also discusses the problem of environmentally-conditioned parasitoses, particularly their etiological factors associated with food contaminated with invasive forms (Trichinella sp., Toxoplasma gondii). The analysis also concerns the presence of developmental forms of geohelminths (Toxocara sp.) and ectoparasites (ticks), which are vectors of serious human diseases (Lyme borreliosis, anaplasmosis, babesiosis), in the environment. Mycological topics contains rare cases of mycoses environmentally conditioned (CNS aspergillosis) and transmissions of these pathogens in a population of hospitalized individuals, as well as seeking new methods used to treat mycoses.

Type 2 diabetes risk factors among the unemployed

Unemployment is a significant social problem which has numerous negative health consequences. Findings of numerous researches provide  data on the range of the consequences of unemployment with risk of health deterioration being one of the most important. Unemployment remains an area of interest for the following fields of science: psychology, economics, sociology. More and more researches prove that there is a link between the increase of disease occurrence and death rate and long-term unemployment. Both in Poland and all over the world a steady increase in the incidence of type 2 diabetes is being observed. World Health Organization (WHO) estimates that, taking into consideration present dynamics of changes, in 2025 there will be 300 million people diagnosed with diabetes. That is why a diabetes epidemic is such a topical issue, and according to some epidemic data this epidemic is starting to reach Poland as well. Data provided by IDF (International Diabetes Federation) show that in Poland the percentage of diabetes patients is 9,1% of the whole population and it is a little higher than the European average (8,6%). It is predicted that by 2015 the percentage will rise to 11%. Identification of adverse health behaviors and introducing actions promoting health in a given population group can prove to be beneficial for the present condition of members of a given group as well as decrease the risk connected with the occurrence of diseases associated with the progress of civilization in the future. The aim of the article is to indicate type 2 diabetes risk factors among the unemployed in Poland.
 Background: The coexistence of cystic fibrosis (CF) and celiac disease (CD) has been reported. To our knowledge there is no study directly comparing the incidence of CD in CF patients to that in the general population at the same time. There is no published data on genetic predisposition to CD in CF patients either. Therefore, in the present study we aimed to assess the genetic predisposition to CD and its incidence in CF patients comparing it to data from the general population. Patients and methods: Two hundred eighty-two CF patients were enrolled in the study. In 230 CF patients the genetic predisposition to CD (the presence of HLA-DQ2/ DQ8) was assessed. In all CF patients, serological screening for CD was conducted. In patients with positive antiendomysial antibodies (EMA) gastroduenoscopy was offered. Intestinal histology was classified according to modified Marsh criteria. The results of serological CD screening in 3235 Polish schoolchildren and HLA-DQ typing in 200 healthy subjects (HS) were used for comparison. Results: Positive EMA was found in 2.84 % of the studied CF patients. The incidence of proven CD was 2.13 %. The incidence of CD as well as positive serological screening were significantly more frequent in the CF group than in the general population. The frequency of CD-related HLA-DQ alleles in CF and HS did not differ. Conclusions: Genetic predisposition to celiac disease in cystic fibrosis patients is similar to that of the general population. However, our results suggest that cystic fibrosis is a risk factor for celiac disease development.
Actual status of mountain forests especialy, of Norway spruce (Picea abies Karst. (L)) stands is a result of combined action between forest management measures and biotic and abiotic risk factors. In this context, tree growth dynamics is a result of multifactor interaction (competition, disturbance, management practice, climate etc.). Wind damage affect the stand structure and normal ecosystem functions including the relationship between individual tree growth and competition processes. Though, it is recorded a diminishing of productive capacity for affected stands as well as a significant diameter increment for remaining standing trees. The correlation between tree growth (cumulate basal area increment in the last 10 years) and competition (Hegyi and Schutz competition indices) was analysed in stands with different windfall intensity. No relationship between tree growth and competition was observed in highly affected stands. Schutz competition index explains better individual tree growth comparing with Hegyi index in low or no-affected stand by windthrow. The variability of the basal area increment (lnΔɡ10), explained by competition indices, is rather low (<2%) in the stand that had been highly affected by windfall, regardless of the selected competition index. Thus, significant reduction of the stand density related to number of trees (low density), as a consequence of high intensity windfalls, crown competition has a very small influence on the basal area increment comparing with situations met in low (high density) and moderate (medium density) affected stands.
Nitric oxide (NO) synthesized by endothelial nitric oxide synthase (eNOS) enzyme is a critical regulator of cerebrovascular homeostasis. Genetic variability of G894T and intronic 4ab polymorphism in eNOS could affect the expression and activity of eNOS enzyme, modulating NO levels in endothelium. This results in endothelial dysfunction, which can contribute to the pathogenesis of ischemic stroke. The purpose of the study was to evaluate the association of eNOS genetic polymorphisms (G894T and 4ab) with the occurrence of ischemic stroke through various genetic models. Both polymorphisms were genotyped in 120 ischemic stroke patients diagnosed with MRI and other ancillary techniques and 101 control subjects free of neurological abnormalities, using PCR-RFLP technique and direct PCR respectively. The genotypes of both G894T and 4ab variants were found to be in Hardy Weinberg equilibrium for cases and controls. The significant variation was observed in the genotypic and allelic frequencies for G894T polymorphism between cases and controls, indicating the association of G894T variability with ischemic stroke. However, the difference between cases and controls was insignificant for eNOS 4ab polymorphism with regard to genotypic and allelic distribution. Except for recessive model, both dominant (GT/TT vs. GG) and co-dominant (TT vs. GT or GT vs. GG) models indicated nearly two-fold and 1.93 increased risk of ischemic stroke for G894T polymorphism, but none of them suggested the influence of eNOS 4ab polymorphism on ischemic stroke susceptibility. Haplotype analysis revealed the higher frequency of GT-4bb genotype combination in cases as compared to controls, but without significant difference. The study concluded that SNP G894T variant is associated with ischemic stroke and might contribute to ischemic stroke susceptibility in North Indians. However, this outcome needs to be confirmed by studies with large sample size.
Background. Breast cancer is a disease with an increasing incidence, and it originates from several factors. Risk factors of this disease represent a diverse group of parameters, which also include hormonal influences. Objective. The aim of the present study was to determine if there is a relationship between hormonal contraceptives and hormone replacement therapy in patients with diagnosed most common types of breast cancer taking into account the age at which the disease was diagnosed in selected sonographic clinic in the district of Nitra, Slovak Republic. Material and Methods. As a part of an ongoing retrospective study from 2005, a cohort of 300 randomly selected patients aged 25-87 years with diagnosed breast cancer have been monitored in 2012. The age at which breast cancer was diagnosed, and relationship to hormone therapy were analysed based on medical documentation. Results. Among 300 randomly selected patients with a confirmed diagnosis of breast cancer the group at the age of 40-59 was the most numerous (mean age was 53.06±11.25 years). The mean age of 45 patients who were given hormonal contraceptives (HC) was 46.44±7.34 years, whereas the mean age of 82 patients who were subjected to hormone replacement therapy (HRT) was 53.63±6.67 years. In women who took hormonal treatment, breast cancer was diagnosed at the mean age of 50.60±7.56 years, while women who never took HRT or HC, were diagnosed around the age of 53.92±12.35 years. The most common types of cancer were the infiltrating ductal carcinoma and lobular carcinoma. From all patients, the relapse occurred among 14 of them, about 4.86 years later. Only 2 patients had breast cancer on both diagnosed breast. After a surgery, the cancer was observed in 7 patients. The positive family history was confirmed with 34 patients. Conclusions. Nowadays, the breast cancer treatment, especially at early stages, is successful, however, the malignant breast neoplasm remains the most common oncological disease causing the death amongst women. In the assessment of the impact of HC and HRT on breast cancer, it is not possible to provide a clear conclusion, because their influence on the tumour is difficult to verify.
The aim of the study was to identify prognostic factors that would differentiate patients with interstitial lung disease between those with and without a chance to survive until lung transplantation. A retrospective study was performed in patients with interstitial lung disease referred for lung transplantation between September 1999 and April 2005. The analysis included the demographic data, the time from referral to transplantation, the functional tests (FVC, FEV1, FEV1%VC, the PaO2 at rest and after oxygen supplementation via a nasal catheter), the count of NYHA functional classes, the left ventricular ejection fraction (EF), the distance covered during a 6-min walk test, and the pathogens in the respiratory tract. The patients were divided into two groups: Group 1 - lung transplant candidates who survived until the successful procedure and Group 2 - lung transplant candidates who died while on the waiting list. There were statistical differences between the two groups in PaO2 after supplementation (P=0.005), EF (P=0.002), and the 6-min walk distance (P=0.001). It appears that simple functional tests of the cardiorespiratory system may define survival of patients with interstitial lung disease waiting for lung transplantation.
Background: Comprehensive cardiac rehabilitation (CR) is a process designed to restore full physical, psychological and social activity and to reduce cardiovascular risk factors. Fear of movement may contribute to the occurrence and intensification of hypokinesia, and consequently affect the effectiveness of therapy. The aim of the study was to determine the level of barriers of physical activity in patients undergoing cardiac rehabilitation. The relationship between selected determinants (age and health selfassessment) and the kinesiophobia level were also examined. Material/Methods: 115 people aged 40-84 years were examined: 50 females (x = 63.46; SD = 11.19) and 65 males (x = 64.65; SD = 10.59) – patients undergoing cardiac rehabilitation at the Upper-Silesian Medical Centre in Katowice. In the present study, the Polish version of questionnaires: Kinesiophobia Causes Scale (KCS) and Short Form Health Survey (SF-36) were used. Questionnaires were supplemented by authors’ short survey. Results: The patients presented an elevated level of kinesiophobia, both in general as well as in individual components. In women, the kinesiophobia level was higher than in men. The psychological domain was a greater barrier of physical activity than the biological one. Strong, negative correlations of psychological and biological domains of kinesiophobia to physical functioning (SF-36) were noted in women. In the case of men, correlations were weaker, but also statistically significant. Conclusions: 1. Sex differentiates patients in their kinesiophobia level 2. Poor self-assessment of health is associated with a greater intensification of kinesiophobia 3. A high level of kinesiophobia may negatively affect cardiac rehabilitation process
Introduction. Elevated uric acid (UA) is associated with arterial hypertension (AH), obesity, dyslipidemia and insulin resistance. However, its association with body components has not been previously investigated. Objective. The aim of this study was to evaluate the relationship between UA and cardiovascular risk factors, anthropometric parameters and body composition in patients with AH. Materials and method. In 138 patients with AH the following parameters were evaluated: UA, low and high density lipoproteins (LDL-C, HDL-C), triglycerides (TG), fasting glucose (FG), creatinine; body mass index (BMI), waist circumference (WC), fat mass (FM), fat free mass (FFM) and total body water (TBW). Results. Positive correlations were shown between UA and LDL-C (p=0.041), TG (p<0.001), FG (p=0.025) and creatinine (p<0.001) and negative between UA and HDL-C (p<0.001). Significant associations between UA and anthropometric parameters and body components, such as WC (p<0.001), BMI (p<0.001), FFM (p<0.001) and TBW (p<0.001), were also observed. In the multiple regression model, independent predictors of UA concentration were serum creatinine and TBW (R2=0.45; p<0.001). Conclusions. In patients with AH, uric acid was significantly related to cardiovascular risk factors, including obesity. However, the main anthropometric determinant of plasma UA concentration is FFM. The consideration of body composition in the interpretation of UA concentration appears to be justified, but the verification of this hypothesis requires further studies.
Lung cancer risk factor in the immigrant population of an industrial area in Poland was examined in a case-control study. Both, relative and summary risks were calculated with adjustment for the following factors: cigarette smoking, alcohol consumption, atmospheric pollution in the place of residence, occupation, education and age. The multivariate logistic analysis indicated that cigarette smoking was a dominant risk factor. In addition, the following factors were confirmed to be significant lung cancer risk factors in the immigrant population of Upper Silesia (Poland): residence in the area of the greatest ecotoxins' concentration; work as toolmakers or manual laborers and seemingly, as coal-miners; lack of university or secondary education, and vodka consumption.
Evidence suggests that maternal immune activation (MIA) during pregnancy is a risk factor for neurodevelopmental disturbances including autism spectrum disorders (ASDs). Animal models support this linkage and demonstrate that MIA in rodents leads to behavioral alterations in offspring that are characteristic of autism. However, the mechanism by which MIA causes long‑term behavioral deficits is unknown. Investigation of the links between maternal infection during pregnancy, mitochondrial dysfunction, and behavioral alterations in offspring. To induce MIA, pregnant Wistar rats were injected with lipopolysaccharide (LPS; 0.1mg/kg, intraperitoneally) on gestational day 9.5, a time point analogous to the first trimester of human gestation. Brains from adolescent offspring were evaluated for mitochondrial outcomes. Prenatal exposure to MIA led to anxiety and repetitive behavior. Adolescent offspring of MIA dams exhibited up-regulation of pro-inflammatory cytokines, oxidative stress, and disturbances in redox homoeostasis. Moreover, substantial mitochondrial abnormalities were observed. A significant decrease in mitochondrial membrane potential and changes in ATP production could be attributed to a downregulation of complex I and IV. Deregulated bioenergetics of mitochondria were accompanied by impaired mitochondrial dynamics, altered expression of fusion/fission machinery proteins including mitofusin 1 and 2 (Mfn1, Mfn2), Opa1, dynamin related protein‑1 (Drp1), and fission protein 1 (Fis1). We also demonstrated lower expression of the genes coding for PGC1α and TFAM (PPARGC1A and TFAM, respectively) that are responsible for mitochondrial biogenesis. MIA at early gestation leads to long-lasting effects on the mitochondrial bioenergetics, dynamics, and biogenesis in the offspring which can lead to synaptic dysfunction and behavioral abnormalities similar to ASD. Tasks of risk manager in the construction enterprise

The paper shows that phenomena requiring an active attitude of managers, including among other things: creating risk management policy, risk identification, risk measurement, taking actions and control, are the subject of risk management. The tasks of a risk manager in the enterprise refer to a professional interpretation of economic correlations, occurring both outside and inside the enterprise. They consist in assessing information about potential risk, possible ways of lowering its effects, unfavourable for the enterprise, and indicating the advantages coming from taking risky actions. The article contains results of own research among 156 Polish building companies in the area of risk factors of building investment projects. According to opinions of Polish building entrepreneurs, Polish enterprise have biggest problems with the scope, schedule, budget and quality requirements of construction projects.
Anorexia nervosa is a serious and potentially life-threatening eating disorder. Persons, who struggle with this problem, consciously starve and quickly lose weight. It is one of the most common mental disease among young women. The following article is an introduction to the anorexia issue. It presents a definition of anorexia, its historical background, a discussion about its causes and ways of treatment. The knowledge of anorexia as well as its risk factors is essential to take appropriate measures connected with the help for the ill people. Anorexics, who discern their disease and understand related to it dangers for their health and lives, undergo treatment more willingly. The following text shows recent Polish research and discusses the anorexia problem in this country. This article also presents a controversy over polish Pro-Ana movement.
