Intensywny rozwój antybiotykoterapii spowodował ograniczenie zainteresowania terapią probiotyczną. Jednak pomimo poprawy skuteczności terapeutycznej antybiotyków, infekcje przewodu pokarmowego i ich następstwa stanowią poważny problem kliniczny. Dodatkowo stały wzrost liczby antybiotykoopornych patogenów bakteryjnych sprawiły, że terapia probiotyczną przeżywa swój renesans. Podczas gdy celem leczenia antybiotykami jest ograniczenie lub zniszczenie patogénnych szczepów bakterii, Probiotyki prowadzą do jakościowej i ilościowej poprawy składu fizjologicznej floty bakteryjnej, ograniczając w ten sposób naturalny sposób rozwój bakterii patogénnych. Probiotyki zmniejszają częstość występowania biegunek podróżnych, łagodzą przebieg i skracają czas trwania niektórych biegunek bakteryjnych i wirusowych (indukowanych przez Clostridium, difficile, Shigella, Salmonella, enterotoksyczne szczepy Eschericia coli czy rotawirusy), zapobiegają wystąpieniu lub łagodzą przebieg biegunek bakteryjnych po antybiotyko-, chemo- i radioterapii, likwidują lub zmniejszają objawy nietolerancji laktozy, mogą działać leczniczo w encefalopatii wątrobowej, a także normalizują zaburzenia motoryki jelit u osób w wieku podeszłym.

A probiotic is defined classically as a viable microbial dietary supplement that beneficially affects the host through its effects in the intestinal tract. This definition, however, was initially intended for use with animal feed products. For the purpose of human nutrition, the following definition has been proposed: "a live microbial food ingredient that is beneficial to health". Probiotics have been shown to improve lactose digestion by reducing the intolerance symptoms as well as by slowing orocecal transit, mainly due to the increase in the release and activity of ß-galactosidase. Probiotics have been shown boost the immune system by enchancing concentrations of circulating Immunoglobulin A, which correlates with shortened duration of rotavirus-induced diarrhea as well as by stimulating the production of cytokines by blood mononuclear cells. Probiotic Lactobacilli and Bifidobacteria strains decrease the quantity of such fecal microbial enzymes as ß-glucuronidase ß- ghicosidase, nitroreductase, and urease, which participate in the metabolic activation of certain mutagens and carcinogens. Rota virus-induced diarrhea, travellers'' diarrhea, antibiotic-induced diarrhea, lactose intolerance Crohn's disease and possibly colon cancer are the only disorders for which there is evidence of disease reduction from probiotic consumption.
