Wstęp: Rodzinna hipercholesterolemia (FH) jest zaburzeniem dziedziczonym jako cecha autosomalna dominująca, wywołana mutacjami genu receptora LDL, które należą do najczęściej występujących w Polsce defektów genetycznych prowadzących do przedwczesnej choroby niedokrwiennej serca (ChNS). Szacuje się, że heterozygotyczna FH występuje w Polsce u około 76 tys. osób. Cel: ocena stężenia homocysteiny (Hcy), witaminy B12 i kwasu foliowego u osób z FH z mutacją genu LDLR przed i po interwencji dietetycznej. Materiał i metoda: W badaniu uczestniczyły 23 osoby z mutacją genu LDL-R w wieku 18-50 lat. Dwukrotnie pobrano krew przed i po zastosowaniu diety, oceniając stężenie Hcy, witaminy B12 i kwasu foliowego. Również przed i po interwencji żywieniowej przeprowadzono 3-dniowy wywiad żywieniowy oceniający realizację diety. Przygotowano 14-dniowe jadłospisy o różnym zapotrzebowaniu energetycznym: 1500 i 1800 kcal. Poziom spożycia kwasu foliowego ułożono na poziomie 400 µg a witaminy B12 -2,4 µg/dzień. Interwencję żywieniową prowadzono przez 6 tygodni. Wyniki: Średnie stężenie Hcy przed interwencją żywieniową wynosiło 13,75 (±4,26) µmol/l. Najniższe stężenie wynosiło 8,45 µmol/l, a najwyższe 21,05. Po interwencji dietetycznej średnie stężenie wynosiło 12,04 (±3,98) µmol/l, najniższe 6,13 µmol/l, najwyższe 20,45 µmol/l. Średnie stężenie wit. B12 przed interwencją żywieniową wynosiło 366,8 (±144,5) pmol/l. Najniższe stężenie wynosiło 202 pmol/l, a najwyższe 637. Po interwencji dietetycznej średnie stężenia wynosiło 401,5 (±165,4) pmol/l. Najwyższe stężenie wynosiło 802 pmol/l, a najniższe 199. W przypadku kwasu foliowego średnie stężenie przed rozpoczęciem diety wynosiło 6,59 (±2,21) nmol/l. Najniższe stężenie wynosiło 3,83 nmol/l, a najwyższe 10,46. Po diecie średnie stężenie wynosiło 7,07 (±1,68) najniższe 3,33, a najwyższe 9,42. Wnioski: Proponowana dieta wykazuje korzystne tendencje zwłaszcza w przypadku witaminy B12, wpływając na zwiększenie jej ilości u wszystkich badanych. Mimo iż osoby te wymagają często leczenia farmakologicznego, zastosowanie u nich diety może być ważnym elementem terapii, zwłaszcza w oddziaływaniu na homocysteinę, której poziom istotnie się obniżył.

Introduction: Family hypercholesterolemia (FH) is a disorder inherited as a dominant autosomal trait caused by the mutation of the LDL receptor gene which belong to the most common genetic defects leading to premature coronary heart disease. Heterozygous FH has 76 000 people in Poland. Aim: assessment of homocysteine (Hcy), vitamin B12 and folic acid intake in persons with FH with the LDL-R gene mutation before and after dietary intervention. Material and Method: 23 persons with the LDL-R gene mutation took part in the study, aged between 18-50. Blood was taken twice, before and after the application of the B12 vitamin and folic acid. Also before and after the intervention a 3 day diet realization assessment interview was conducted. 14 day menus were prepared with different energy intake requirements in mind: 1500 and 1800 kcal. The level intake of folic acid was 400 g and vitamin B12 - 2.4 µg/day. The intervention lasted for 6 weeks. Results: The average concentration of the homocysteine before the intervention was 13.75 (±4.26) µmol/l. The lowest was 8.45 µmol/1, highest 21.05. After intervention the average concentration was 12.04 (±3.98) µmol/1. The lowest was 6.13 µmol/1, highest 20.45 µmol/1. The average concentration of the B12 vitamin before the intervention was 366.8 (±144.5) pmol/1. The lowest was 202 pmol/1, highest 637. After intervention the average concentration was 401.5 (±165,4) pmol/1. The lowest was 199 pmol/1, highest 802 pmol/1. In the case of the folic acid the average concentration was 6.59 (± 2.21) nmol/1 before the diet. The lowest was 3.83 nmol/1, highest 10.46. After the diet the average concentration was 7.07 (±1.68), lowest was 3.33, highest 9.64. Conclusions: the proposed diet shows beneficial tendencies especially in the case of vitamin B12, inereasing concentration for all the participants of the study. Even though such persons require pharmacological treatment this diet may be an important element in the therapy especially in reference to homocysteine which levels are significantly reduced.
