Cukrzyca typu 2 rozwija się na podłożu insulinooporności i zmniejszającej się funkcji komórek beta. Na podstawie wytycznych Polskiego Towarzystwa Diabetologicznego 2008 ustalono kolejne kroki postępowania we wczesnych zaburzeniach gospodarki węglowodanowej (prediabetes) oraz etapy postępowania farmakologicznego w rozwoju choroby. Należy pamiętać o kompleksowym, wieloczynnikowym postępowaniu w prewencji i spowolnieniu przewlekłych powikłań cukrzycy. W algorytmie postępowania w cukrzycy po raz pierwszy wymieniono leki oddziaływujące na układ inkretynowy (agoniści GLP-1) podawane podskórnie i inhibitory DPP-IV, stanowiące alternatywę leczenia doustnego. Omówiono leki tych grup ustalając ich miejsce w terapii leczenia cukrzycy (agoniści GLP-1 w terapii skojarzonej z lekami doustnymi przy ich nieskuteczności) i inhibitory DPP-IV podawane doustnie w początkowych stadiach cukrzycy w monoterapii i leczeniu skojarzonym z innymi lekami doustnymi.

Type 2 diabetes is developing on base of insulin resistance and decreasing function of beta cells. The successive steps of behavioral intervention in the early stage of carbohydrate metabolism (prediabetes) and strategy of pharmacological intervention in progress of disease are determined on the basis of Polish Diabetes Association Guidelines. It is important to introduce comprehensive, multifactor modification in prevention to delay chronic complications of diabetes. Drugs with action via incretin system was listed for the first time in this algorithm: incretion mimetics (GLP-1 agonists receptor) administered by injection and DPP-IV inhibitors, which are presenting an alternative way of oral treatment. The drugs of this groups were discussed to determine their place in the therapy of diabetes (GLP-1 agonists in combine therapy with oral drugs in case of their failure) and inhibitors of DPP-IV orally administered in initial studies of diabetes, in monotherapy or therapy associated with other oral drugs.
